Gulf Breeze Apothecary works with licensed prescribers and authorized practice teams on requests involving patient-specific compounded prescriptions. Every provider relationship, service state, prescription workflow, and requested capability is reviewed before approval.

Provider qualification

Who can request a provider relationship

Licensed prescribers and authorized staff from medical, dental, and veterinary practices may request a consultation. Submission does not establish an account or confirm service eligibility.

Requests may involve catalog-listed categories such as dermatology, pediatric, hormone, women's health, men's health, and veterinary compounding. Specific formulations and dosage forms are confirmed only after review.

Approved practices receive current instructions for submitting patient-specific prescriptions through an accepted channel.

Formulation consultation

A pharmacist may discuss dosage-form and formulation questions after the practice and request are reviewed.

Preparation and coordination

Accepted prescriptions are evaluated and prepared according to the pharmacy's applicable requirements. Timing is confirmed case by case.

The provider team remains available for approved workflow and formulation questions.

Providers may request pharmacist consultation about formulation and dosage-form questions. Availability, prescription submission methods, sterile preparation, and timing are confirmed for each approved relationship and request.

Existing approved providers may use the pharmacy's LifeFile provider portal. New practices should complete the consultation form before attempting portal access.
